Redbridge ‘One Panel’
What is the ‘One Panel’?
The Redbridge ‘One Panel’, launched in February 2023, provides a forum to bring together referrals for cases to be considered for review. These include Safeguarding Adult Reviews (SARs), Domestic Homicide Reviews (DHRs) and Child Safeguarding Practice Reviews (CSPRs). The Panel will also undertake the important role of monitoring recommendations of completed reviews and ensuring that learning is shared across Redbridge.

The Guidance covers the purpose and roles and responsibilities of the Panel, plus information on the process for making a referral, membership and chairing arrangements.
Below is an introductory guide on Domestic Homicide Reviews (DHRs) which will be of help to professionals who have not been involved in this type of review before.
Referrals
Referrals to the ‘One Panel’ should be made using the form below:

It is important that a referral, once identified by an agency/professional as being required, is received into the Panel Secretariat in a timely way. Prior to a referral being heard at one of the monthly Panel Meetings, a scoping exercise needs to be undertaken which can take approximately two weeks. For maximum learning to be achieved from a Case Review, and of course any identified immediate safeguarding actions to be taken, it is important that referrals are not delayed.
